New Patient Registration

You must reside within the practice catchment area. If you would like to apply for new patient registration please fill out the registration form provided at reception. On return of this form you MUST provide proof of address and photographic ID.

At the same time please provide a medical summary report including significant medical past , current medications and vaccination history.

Your forms are sent to BSO registration office and placed on A WAITING LIST which can take weeks to run all the necessary checks in order to confirm permanent NHS registration.

Registration with the practice

We are pleased to welcome new patients living in the Practice Area.

Please bring your previous NHS Medical Card to reception, to complete a New Patient Questionnaire and make an introductory appointment with one of the Doctors.

If you cannot find your previous medical card, our reception staff will be able to give you the necessary forms to be completed.

If you have never been registered in the NHS before and are arriving from another EU country- the necessary forms are provided in reception.

If you have never been registered in the NHS and have come from outside the EU- it is necessary to provide proof of identity and NHS care eligiblity. A copy of your passport and UK Visa are required.

The New Patient Questionnaire allows the practice to start a Health Record.
