Every week a number of patients fail to attend booked appointments. Please let us know as soon as possible if you cannot keep your appointment. It means we can give it to someone else! Thank you.

Whooping Cough Vaccination

This is advised for all pregnant women from or as soon after the 16th week of pregnancy as possible. Please make an appointment with the Practice Nurse.

Please do not book in for RSV vaccination during pregnancy , this should be arranged at local vaccination centres. contact your midwife for more info. 

 

New Patient Registration

You must reside within the practice catchment area. If you would like to apply for new patient registration please fill out the registration form provided at reception. On return of this form you MUST provide proof of address and photographic ID.

At the same time please provide a medical summary report including significant medical past , current medications and vaccination history.

Your forms are sent to BSO registration office and placed on A WAITING LIST which can take weeks to run all the necessary checks in order to confirm permanent NHS registration.
